Yes, if deemed medically necessary by the provider based on your clinical evaluation and the severity of your presenting symptoms.
However, in our efforts to provide honest, accurate, and substantial information for any disability-related accommodations including but not limited to Family and Medical Leave Act (FMLA) paperwork or short-term disability paperwork, you must be a patient at the practice who has received mental health services consistently (attending at least biweekly or monthly appointments) for at least 6 months.
Amethyst Holistic Health, PLLC is unable to complete any paperwork for long-term or permanent mental disability.
If you need to reschedule or cancel your appointment, please contact your provider or self-schedule via the Simple Practice patient portal to reschedule and/or cancel your appointment at least 48 hours before your scheduled appointment time.
At the time of your appointment being scheduled, your time slot is reserved for you and this prevents other patients from being able to schedule with the provider during this time.
Therefore, missed appointments and late cancellations are subject to a $100 fee.
Business hours are Tuesdays, Wednesdays, and Thursdays between 9:00 am – 4:00 pm. Appointments scheduled outside of business hours may be available upon request.
Please allow at least 48-72 hours for responses or appointment requests outside of business hours. If you are a current patient, please contact the provider via the Simple Practice patient portal for any medication refills or concerns.
Amethyst Holistic Health does not offer after-hours answering services or emergency care.
If you or someone you know are experiencing a mental health crisis, having suicidal or homicidal thoughts, or any other emergency situation please call 9-1-1 or 9-8-8 and report to your closest emergency department for evaluation.
